SPARK REGISTRATION
STEP 1: Register your child (this form is needed to participate in any SPARK programming)

When you click submit, you will be redirected to our waiver packet which also needs to be signed.
NOTE: If you have submitted this form for your child in the past, please do not fill it out again. Please contact us by phone or text at 715.255.0509 and we can send you a link to update your child's record.
STEP 2: Reserve a Spot!
We have a limit on the number of youth we can accommodate each day.
Please reserve a spot for today or tomorrow using the link below each day you would like to come to SPARK!
If you don't see the day you're looking for, we're already full for that day. Check back tomorrow!
